The clinical setting in which I used to work was a correctional setting. While working in a correctional facility, I encountered several inmates who had HIV. It was this atmosphere and the experiences I had that inspired me to create a proposal for change.

Within this system, I found that staff were not getting much prevention and education, and inmates were not getting any prevention education at all. During my time working in correctional facilities and meeting HIV-positive people. One evening, during a medication line run, a 17-year-old boy walked up to the medication window. His medication was a barrage of HIV antivirals. I was shocked and found the situation very disturbing. I assumed it bothered me because I, too, have a child at that age.

This whole scenario with the HIV-positive young man made me feel that I had somehow dropped the ball on him and his situation and let this young man down. I often wondered if this young man had been instructed on prevention and education before he was diagnosed with HIV. As a health care provider and parent in this situation, I personally felt a responsibility to do what I could to educate people about HIV prevention and education. After that initial shock, I encountered several other people who crossed my path between the ages of 17 and 21 who were also HIV-positive.

I also found that staff and inmates were in varying degrees of need of a primary prevention and education plan. As a fully staffed medical unit, there was very little or no patient education. I believe it is very important to implement a prevention and health management program for both inmates and staff. The goal of such a program for staff would be to ensure that inmates/patients are educated about their status. Implementing a prevention and education program will give inmates an understanding of their diagnosis and promote medication compliance in addition to giving inmates/patients the opportunity to participate in their health management. This can be accomplished by adhering to medication and avoiding risky behaviors such as sexual activity or getting tattoos, or any other risky behaviors. As for medical and security staff, deepening their knowledge base with a thorough training system and a system that promotes prevention will reduce the risk of staff and inmate exposure.
